Makeup class request form

*Please note that Make-up classes will be capped at FIVE per THREE MONTHS from the end of the grading to the start of the next grading. (excluding 2weeks Christmas Break, Public Holidays, your long holidays)

You can have MAKEUP class only when:

  • You are sick   (Sick days can be claimed for ONE DAY without a medical certificate)
  • Public holiday (except Christmas Holidays) falls on your Training day
  • UMA EVENTS (UMA championships or Water Fight) fall on your Training days.

The form must be submitted no later than 48 hours after the missed class, otherwise your request will be declined. Please discuss with the front desk coordinators about MAKEUP class if you are not sure. Thank you.
